I have been wanting to make this card shape ever since I purchased it from the Silhouette Store, you can find it here. It's actually sized to hold a gift card, but I made mine much smaller; it's more of a note card size.
I used stamps from Warm Hearts to decorate the cozy and then ran it through a crimper. The organza ribbon was wrapped around the cozy and then I attached it with some boosters.Once adhered, I added some glossy accents to the holly berries and to the heart in the sentiment.
